Ramelteon, sold under the brand name Rozerem among others, is a melatonin agonist medication which is used in the treatment of insomnia. It is indicated specifically for the treatment of insomnia characterized by difficulties with sleep onset. It reduces the time taken to fall asleep, but the degree of clinical benefit is small. The medication is approved for long-term use. Ramelteon is taken by mouth. Side effects of ramelteon include somnolence, dizziness, fatigue, nausea, exacerbated insomnia, and changes in hormone levels. Ramelteon is an analogue of melatonin and is a selective agonist of the melatonin MT1 and MT2 receptors. The half-life and duration of ramelteon are much longer than those of melatonin. Ramelteon is not a benzodiazepine or Z-drug and does not interact with GABA receptors, instead having a distinct mechanism of action. Ramelteon was first described in 2002 and was approved for medical use in 2005. Unlike certain other sleep medications, ramelteon is not a controlled substance in nearly every country and has no known potential for misuse.

=== Australia === Across most of the continent, El Niño and La Niña have more impact on climate variability than any other factor. There is a strong correlation between the strength of La Niña and rainfall: the greater the sea surface temperature and Southern Oscillation difference from normal, the larger the rainfall change. During El Niño events, the shift in rainfall away from the Western Pacific may mean that rainfall across Australia is reduced. === Pharmacoperones === Current research is looking into pharmacoperones, or chemical chaparones that promote the shuttling of mature Gonadotropin-releasing hormone receptor (GNRHR) protein to the cell surface, leading to a functional protein. Gonadotropin-releasing hormone receptor function has been shown to be deleteriously effected by point mutations in its gene. Some of these mutations, when expressed, cause the receptor to remain in the cytosol. An approach to rescue receptor function utilizes pharmacoperones or molecular chaperones, which are typically small molecules that rescue misfolded proteins to the cell surface. These interact with the receptor to restore cognate receptor function devoid of antagonist or agonist activity. This approach, when effective, should increase therapeutic reach. Many substances can influence the P-450 enzyme mechanism. Drugs interact with the enzyme family in several ways. Drugs that modify cytochrome P-450 enzyme are referred to as either inhibitors or inducers. Enzyme inhibitors block the metabolic activity of one or several P-450 enzymes. This effect usually occurs immediately. On the other hand, inducers increase P-450 activity by increasing enzyme production, or, in the case of CYP2E1, preventing degradation in the proteasome. There is usually a delay before enzyme activity increases.

The odds of clinical improvement at day 15 was 30% higher in the baricitinib plus remdesivir group versus the placebo plus remdesivir group. For all of these endpoints, the effects were statistically significant. The EUA was issued to Eli Lilly and Company. In November 2020, the World Health Organization (WHO) updated its guideline on therapeutics for COVID-19 to include a conditional recommendation against the use of remdesivir, triggered by results from the WHO Solidarity trial. In November 2020, the FDA issued an emergency use authorization (EUA) for the combination of baricitinib with remdesivir, for the treatment of suspected or laboratory confirmed COVID-19 in hospitalized people aged two years of age or older requiring supplemental oxygen, invasive mechanical ventilation, or extracorporeal membrane oxygenation (ECMO).

=== Challenges === Despite its promise, relatively little silk has reached the clinic, and the reasons are specific. Foremost among them is variability. Silk is a farmed natural product, so its composition shifts with the silkworm strain, its diet and its rearing conditions, and this batch-to-batch variation is difficult to reconcile with the strict specifications required of a medical device. Processing introduces further variation: the duration of exposure to the dissolving salt and the amount of heat applied both affect the molecular weight of the regenerated protein, and with it the strength and degradation of the final product. Sterilisation presents a further difficulty, since standard autoclaving alters the structure of the protein, and a gentler method must be identified and validated for each format. Controlling the degradation rate precisely, scaling laboratory processes to industrial volumes, and meeting the regulatory requirements for an implanted device are each demanding in their own right. These engineering and manufacturing obstacles, rather than any deficiency in the biological performance of silk, are the main reason that most of it remains in the laboratory.
=== Regime type and polarization === Hybrid regimes are more vulnerable to coups than very authoritarian states or democratic states. A 2021 study found that democratic regimes were not substantially more likely to experience coups. A 2015 study finds that terrorism is strongly associated with re-shuffling coups. A 2016 study finds that there is an ethnic component to coups: "When leaders attempt to build ethnic armies, or dismantle those created by their predecessors, they provoke violent resistance from military officers." Another 2016 study shows that protests increase the risk of coups, presumably because they ease coordination obstacles among coup plotters and make international actors less likely to punish coup leaders. A third 2016 study finds that coups become more likely in the wake of elections in autocracies when the results reveal electoral weakness for the incumbent autocrat. A fourth 2016 study finds that inequality between social classes increases the likelihood of coups. A fifth 2016 study finds no evidence that coups are contagious; one coup in a region does not make other coups in the region likely to follow. One study found that coups are more likely to occur in states with small populations, as there are smaller coordination problems for coup-plotters. In autocracies, the frequency of coups seems to be affected by the succession rules in place, with monarchies with a fixed succession rule being much less plagued by instability than less institutionalized autocracies.
The siege of Coimbra in 1064 or the definitive conquest of Coimbra by Christian forces took place in 1064, from January to July; it ended on 9 July 1064, a Friday, when the king, Ferdinand I of Leon, captured the city from the Muslims. The city of Coimbra had previously been taken from the Christians by Almanzor (or al-Manṣūr) in 987. When the Fitna of al-Andalus broke out and the Umayyad Caliphate of Cordova fragmented into numerous taifa states, Dom Sesnando Davides, lord of Tentúgal proposed the conquest of the city to Ferdinand of Leon. Ferdinand thus began preparations for the campaign in December 1063, after receiving the remains of Saint Isidore of Seville in León. Before he moved against Coimbra however, he made a pilgrimage to Santiago de Compostela. Having prayed there before the tomb of the apostle, St. James of Compostela, for three days and made several donations to the church there, he departed accompanied by his wife Dona Sancha, his sons, the Bishop Crescónio of Santiago, Bishop Vistruário of Lugo, Bishop Suário of Mondonhedo, Bishop Sesnando of Portugal, the abbot Ariano of Cela Nova, abbot Pedro of Guimarães, and a large number of nobles. They advanced through a coastal road that linked Santiago de Compostela to Iria Flavia to Braga, Porto and Coimbra. They arrived before the walls of the city on 20 January 1064. The Muslim garrison fiercely resisted the Christian attacks. The supply situation of the Christians was not favourable and Ferdinand even pondered lifting the siege.
==== Action and reference level ==== WHO presented in 2009 a recommended reference level (the national reference level), 100 Bq/m3, for radon in dwellings. The recommendation also says that where this is not possible, 300 Bq/m3 should be selected as the highest level. A national reference level should not be a limit, but should represent the maximum acceptable annual average radon concentration in a dwelling. The actionable concentration of radon in a home varies depending on the organization doing the recommendation, for example, the EPA encourages that action be taken at concentrations as low as 74 Bq/m3 (2 pCi/L), and the European Union recommends action be taken when concentrations reach 400 Bq/m3 (11 pCi/L) for old houses and 200 Bq/m3 (5 pCi/L) for new ones. On 8 July 2010, the UK's Health Protection Agency issued new advice setting a "Target Level" of 100 Bq/m3 whilst retaining an "Action Level" of 200 Bq/m3. Similar levels (as in the UK) are published by Norwegian Radiation and Nuclear Safety Authority (DSA) with the maximum limit for schools, kindergartens, and new dwellings set at 200 Bq/m3, where 100 Bq/m3 is set as the action level.
Methylphenidate is a psychostimulant drug that is often used in the treatment of ADHD and narcolepsy and occasionally to treat obesity in combination with diet restraints and exercise. Its effects at therapeutic doses include increased focus, increased alertness, decreased appetite, decreased need for sleep and decreased impulsivity. Methylphenidate is not usually used recreationally, but when it is used, its effects are similar to those of amphetamines. Methylphenidate acts as a norepinephrine–dopamine reuptake inhibitor (NDRI), by blocking the norepinephrine transporter (NET) and the dopamine transporter (DAT). Methylphenidate has a higher affinity for the dopamine transporter than for the norepinephrine transporter, and so its effects are mainly due to elevated dopamine levels caused by the inhibited reuptake of dopamine, however increased norepinephrine levels also contribute to various of the effects caused by the drug.
